NEUROTRAUMATISME PAR ROSTRE D’ESPADON

Abstract

RESUME Nous rapportons deux cas de neuro-traumatisme par un objet insolite : le rostre d’espadon. Il s’agit de deux patients âgés de 19 et 36 ans, sans antécédents pathologiques, qui ont été admis dans le service de neurochirurgie du CHUN de Fann. Le premier avait présenté une plaie pénétrante crânio-cérébrale négligée siégeant au niveau frontal gauche et le second des lombalgies invalidantes dans un contexte fébrile secondaire à une plaie pénétrante du rachis de niveau L4. Chez nos deux patients, le traumatisme faisait suite à des coups et blessures avec un rostre d’espadon. Le bilan radiologique avait mis en évidence pour le premier un abcès cérébral de siège frontal avec présence d’un corps étranger intra cérébral et pour le second une fracture du pédicule gauche et du corps vertébral de L4. Nos deux patients ont bénéficié d’une prise en charge chirurgicale. Mots clés : neuro-traumatisme – abcès – rostre d’espadon ABSTRACT We report two cases of neuro-traumatism by an unusual object: the swordfish rostrum. Two patients 19 and 36 years old, with no history of pathology, were admitted to the Fann CHUN neurosurgery department. The first had a neglected cranio-cerebral penetrating wound in the left frontal area and the second had lower back pain in a febrile context secondary to a penetrating wound of the L4 spine. In both of our patients, the trauma was a result of assault by a swordfish rostrum. For the first patient, the radiological assessment revealed a cerebral abscess of frontal seat with presence of an intracerebral foreign bodyand for the second one, a fracture of the left pedicle and the vertebral body of L4. Both of our patients were treated surgically.
