Un cas de tératome ovarien mature géant au CHU MEL de Cotonou (Bénin) : difficultés diagnostiques et qualité de la prise en charge chirurgicale

Abstract

Résumé Le tératome ovarien mature géant est une tumeur bénigne d’origine germinale et pouvant contenir des éléments provenant des trois feuillets primordiaux de l’embryon. Nous rapportons un cas de tératome ovarien géant, d’évolution rapide révélé par l’aménorrhée, la douleur et des signes de compression abdominale chez une nulligeste de 23 ans sans antécédent particulier avec un état général conservé et une biologie normale. Le diagnostic préopératoire était un tératome immature devant un bilan radiologique. Une annexectomie par laparotomie suivie d’une histologie complète ont permis d’affirmer le caractère bénin de la tumeur. Cette chirurgie radicale s’est révélée excessive au regard des données de la littérature. L’évolution a été favorable mais le pronostic fonctionnel reste réservé. L’effort d’une chirurgie conservatrice à travers une kystectomie est souhaité devant la tumeur de l’adolescente et jeune fille. Abstract The giant mature ovarian teratoma is a benign tumor of germinal origin and may contain elements from the three primordial layers of the embryo. We report a case of giant ovarian teratoma, rapid evolution revealed by amenorrhea, pain and signs of abdominal compression in a 23-year-old nulligest with no particular antecedent with a conserved general state and normal biology. The preoperative diagnosis was an immature teratoma with radiological assessment. Laparotomy adnexectomy followed by complete histology confirmed the benign nature of the tumor. This radical surgery proved excessive in light of the literature data. The evolution has been favorable but the functional prognosis remains reserved. The effort of a conservative surgery through a cystectomy is desired in front of the tumor of the teenager and girl. Keywords; mature ovarian teratoma, diagnosis, surgery, Benin Republic
